Community Health Assessments are key tools for public health departments and hospitals to understand specific community needs and strengths, develop programming to fill gaps, and provide data to policymakers. These assessments are central to our business portfolio, and we work closely with public health and hospital leaders to design an assessment process that works best for their community. We prioritize making data accessible, and we believe that community-gathered data should be returned to the community in a way they can use and understand.
Our assessment package includes:
Quantitative data workbook comprised of more than 250 health indicators that we develop and leave with the client to build upon for their next assessment
Community survey to better understand community-identified values, gaps, and strengths
A comprehensive qualitative approach, which includes stakeholder interviews and community-based focus groups
Full color final report, which includes thoughtful data visualizations and infographics that can be used independently in other reports, presentations, and social media

Our approach builds on the Community Health Assessment (CHA) we have conducted with you, or that you already have completed, and develop a Community Health Improvement Plan (CHIP). This includes a 4-step process that follows MAPP 2.0 guidelines and prioritizes sharing data and outcomes with the community throughout the CHIP process.

We facilitate community conversations, provide training for public health and community based teams, and help small public health departments build capacity around assessment and data collection.
